Forms to Complete After Receiving Notification of the Date of Practice Inspection

All offices to be inspected in 2018-2019 (being inspection numbers commencing with I18 or R18, followed by four digits) will receive a letter confirming the date of the practice inspection visit (or, for an inspection being conducted as a desk inspection, the timeline for submission of requested client files and other information to CPA Ontario) and requesting the completion of certain forms.   Those forms will be updated for 2018-2019 and posted to this web page prior to May 1, 2018; our confirmation letters will not be sent prior to that date.  Offices should note that only the updated forms will be accepted by the inspectors; forms which offices might have downloaded and saved in respect of previous inspections are no longer valid.
